Planning a Compassionate Transition 

During this time we work together to plan and document important elements related to the dying process such as: 
- sacred/special items within view 
- preference for location 
- level of medical intervention 
- vigil requests (people, scents, music) 
- design personalized visualization and mindful exercise
- review comfort with touch 
- after death rituals (such as bathing the body) 
- legacy projects 
-address worries and concerns

Vigil

As a person moves into the active stages of dying, vigil support includes: 
- bedside presence 
- visualizations, mindfulness and other calming practices
- education around the dying process 
- ensuring compassionate plan elements are implement (as much as possible) 
- supporting caregivers and loved ones 
- communicating with medical staff regarding family wishes and the compassionate plan

Grief and Legacy 

Generally this work begins 3-6 weeks after the person has died and includes:
- education around the grief process 
- retelling and reprocessing of the dying process/experience 
- resources and bereavement referrals 
- sharing, planning and/or continuing a legacy project 
- reflection 
- closing ritual
